Bug Triage SOP

For each report:

1. Restate expected vs actual.
2. Extract repro steps; mark missing info.
3. Severity: S0 blocker / S1 major / S2 minor / S3 polish.
4. Suggest component owner from CODEOWNERS-style map.
5. Open or update issue with labels; link related reports.

Do not claim root cause without evidence.
